The RN Case Manager acts as the coordinator of the health care team for individual patients in order to maintain the proper linkages within a continuum of care for best patient outcomes. The RN Case Manager administers medication and treatments, maintains and submits the clinical record for each patient and provides progress notes, as needed.
Provide quality patient care to homebound clients following established professional standards of care, agency guidelines, policies and procedure.
Responsible for the implementation and evaluation of individual patient plan of care.
The Licensed Nurse Practitioner provides nursing care under the supervision of a Registered Nurse and according to the plan of care derived from physician’s orders and Registered Nurse assessment.
A licensed Occupational Therapist performs patient testing, measurements and evaluations. Based on the findings, the OT prepares a written treatment program establishing or revising specifics of the treatment program. Instructs patient, family and other healthcare team members in appropriate phases of occupational therapy.
Speech therapists both identify and treat communication problems related to language and speech. Working with patients of all ages, speech-language pathologists seek to improve communication skills through vocal exercises and cognitive therapies. Patients under going treatment with a speech therapist may have an articulation, language, fluency, or resonance disorder, which prevents them from either being understood or understanding others. Our Speech therapists visit both hospitals and private settings and most therapists work full-time.
A licensed Physical Therapist performs patient tests, measurements and evaluations, such as range of motion and manual muscle tests, gait and functional analyses and body parts measurements. The PT records, analyzes findings to aid in establishing or revising a patient-specific treatment program. The PT instructs patient, family and other healthcare team members in appropriate phases of physical therapy.
Within an assigned territory, on a daily basis, a Community Liaison establishes, maintains and enhances professional relationships with healthcare providers including physicians, skilled nursing facilities, assisted/supportive living communities, independent living communities, hospitals, social workers, discharge planners, community/civic organizations, senior advocate agencies and other home care agencies who influence or make referral decisions.
A Certified Nurse Aide is a certified member of the Home Health Care Team who assists the patient with the tasks of daily living as outlined in a written care plan or agreement that is established by a Registered Nurse Case Manager. Certified Home Health Aides are supervised by the RN Case Manager or LPN.
